We're offering convenient flu shots to help protect you during the flu season. Most insurance plans cover the cost, making it easy for you to get vaccinated. For those without insurance, the flu shot is available for just $25, ensuring everyone has access to this essential service. No appointment necessary. Disability Parking Placard / License Plate 

Begin the process of applying for a disability parking placard or license plate with us. Simply download the form and fill out sections A-C.  Upon completion, bring the form and any supporting documentation (insurance card, driver's license, proof of address) on any Thursday, from 10 to 6. 

THC Cards

You can easily start the process of applying for a THC Card with us. Download the form here, fill out the patient and caregiver information (if applicable), print it, and bring it with you on any Thursday, from 10 to 6. The cost is only $99.
